Statistics of the SVDPC Senior Services Programs

During the 2009/2010 year (7/1/09-6/30/10):

• 125 senior clients were served in the program, with 1,027 client visits during the year.
• Due, in part, to our case management support for senior clients, 96% of our clients were able to continue living independently in their homes during the year.
• St. Vincent’s is the primary service provider of support services for our seniors clients(1).
• 1,640 wellness check and case management services were provided to senior clients.
• Senior clients received companion services to relieve isolation and loneliness. During the year, there were 1,496 companion visits.
• Over 300 transportation and advocacy services were provided for medical appointments and medical benefits. Over 100 transportation and assistance services were provided for grocery shopping.
