Senior Services
Helping Low Income Seniors
St. Vincent de Paul Center’s Senior Services program provides comprehensive services for low-income, homebound seniors who are at-risk on the north side of Chicago. Service for this program begins with an in-person assessment of the individual senior’s needs to determine how we can best be of assistance. The Senior Services program fosters a holistic approach to living by nurturing the spiritual, emotional and physical needs of our senior clients.
Program services include:
- Comprehensive case management
- Transportation and escort for grocery shopping and medical appointments
- Companion services
- Resource and referral services and advocacy services
Senior Services is committed to serving older adults, age 60 and over, who are especially frail and vulnerable due to any combination of the following:
- Lack of family/friend support
- Limited income and assets
- Chronic illness
- History of abuse, neglect, or exploitation
- Visually-impaired and mobility-limited
- Lack of accessible and appropriate transportation
Most of our senior clients do not have family or friends to advocate for them, help them solve problems, or ensure that they have adequate medical care and safe, affordable housing that is appropriate for their physical capabilities. The program provides coordinated services that are essential for older adults to live independently in their own homes, to manage health care needs, to experience emotional support and to participate in their community.
The Senior Services program collaborates with many area hospitals and home care agencies, and private and public agencies and organizations to ensure that the needs of older adults are appropriately met. The Senior Services program also accepts referrals from, and works closely with, the Chicago Department of Family and Support Services/Senior Services division, and our work is funded in part by them.
Continuity of Care
Continuity of care is provided through monthly phone calls and in-home well-being checks, friendly visiting from case managers and volunteers, and coordination with other agencies.
